This section describes Kansas Rehabilitation Services, Services for the Blind, Title VII, Chapter 2 grant, Kansas Seniors Achieving Independent Living (Kan-SAIL) Project.

Kan- SAIL OBJECTIVES  

OBJECTIVE 1  

Steering committee: Access to the DSB Advisory Committee to assist in identification of barriers that prevent older individuals who are severely visually impaired.

1.1      Consultation will be done quarterly. Evaluation Criteria: Input from the Committee   reflected in grant management.
Objective Leader: Project Coordinator 

1.2       Yearly consultation with the KRS Council and Statewide Independent Living Council.  Evaluation Criteria: Input from the two Councils reflected in grant management.

OBJECTIVE 2  

Recruitment: Employ qualified professional staff in accordance with the State Affirmative Action Plan and Civil Service Regulations.  

2.1      Emphasize recruiting Seniors, persons of ethnicity and persons with disabilities by contacting minority organizations, minority and disabled student placement offices at universities and colleges, Senior organizations and consumer groups for the blind.
Evaluation Criteria: Staff reflect diversity.

2.2      Provide staff with reasonable accommodations including providing all written material in Media of Choice.  Evaluation Criteria: No EEO complaints with a finding of fault.

OBJECTIVE 3  

Training and Orientation: Provide ongoing in-service training to Kan-SAIL ΙI staff. Orient KRS and SRS staff to Kan-SAIL ΙI.

3.1      Provide media of choice and E-mail to staff monthly. Present programs at DSB statewide meetings.Evaluation Criteria: Feedback from staff indicates an understanding and scope of the grant and reflects collaboration. 

3.2       Present information on Kan-SAIL II at KRS District Manager meetings.
   
         Evaluation Criteria: Two documents presentations a year.

3.3       Orient appropriate staff in SRS management areas beginning in the Third Quarter of Grant Year One.  Priority will be given to areas with the largest population of Hispanics, Blacks, American Indians, Asians or Pacific Islanders.
Evaluation Criteria: Feedback and presentation Evaluations indicate understanding of the scope of the grant.

OBJECTIVE 4  

Community-Based Outreach Services: Provide 45 community-based services through Outreaches using a mobile training team model.  Independent living skills training will include, but not be restricted to, orientation and mobility counseling and self-advocacy. If the client speaks English as a second language or is hearing-impaired, qualified interpreters will be provided.

4.1       Focus on taking Outreaches to communities with high populations of the under served target populations of Hispanics, Blacks, American Indians, Asians or Pacific Islanders.
Evaluation Criteria: Ten percent served per year from target populations.

4.2      Provide individualized in-home independent living teaching as a viable option to participation in the Outreaches or the RCB Senior Programs by using strategies for providing state-of-the-art teaching of independent living skills.
Evaluation Criteria: Client Exit Evaluations are positive.

4.3       Provide three Senior Week Programs a year at the RCB.
            Evaluation Criteria: 150 clients and 45 significant others are served during the grant.

OBJECTIVE 5  

Low Vision Services: Provide low vision assessments at the RCB, in client homes, at Senior Centers, and during Outreaches. 

5.1       Provide low vision assessments to identify the need for adaptive aids or equipment.    Provide low vision aids and training.  Seek alternative funding through VR or other sources such as Lions Clubs.  Provide follow-up to assure low vision aids are  being used properly and frequently with re-training provided, if necessary.
Evaluation Criteria: Evaluations reflect satisfaction with service. Tracking system documents frequency of follow-up.

5.2       Refer clients to ophthalmologists or optometrists for prescriptive aids or for low vision examinations.  Providing training and follow-up for low vision aids.
Evaluation Criteria: Surveys of ophthalmologists and optometrists about services and cooperation of Kan-SAIL II are positive.

OBJECTIVE 6  

Employment: Refer clients who are interested in employment to the DSB vocational rehabilitation program.

6.1      Consult with the VR District Manager and refer clients to the local DSB Rehabilitation Counselor within ten working days.
Evaluation Criteria: Clients records and Client Exit Evaluations reflect satisfaction.

OBJECTIVE 7  

Services for Significant Others: Establish special programs during Outreaches to assist significant others to cope and understand the emotional and practical day-to-day changes in their lives and their family members lives beginning in the Third Quarter of the First Year Grant.

7.1       Provide Significant Others Programs during Outreaches, RCB Senior Week Programs, and on an individual basis.
Evaluation Criteria: Data on programs and contacts documented.

OBJECTIVE 8  

Special Targeted Services to American Indians: Develop referrals from this under served population.  Promote a new knowledge base of the rehabilitation process for American Indians and encourage dialogue and collaboration with Indian Health Services, Bureau of Indian Affairs, and enrolled and non-enrolled American Indians with individual tribes and Tribal Councils.

8.1       One documented contact with the Indian Health Services, the Bureau of Indian Affairs, and representatives of the four American Indian tribes to develop referrals each quarter.
Evaluation Criteria: Documentation of one contact with Indian Health Services and one contact with each of the four Tribal Councils each quarter.
Objective Leader: Minority Coordinator 

8.2       Collaborate with representatives of American Indian Tribal Councils, Indian Health Services and Administration on Aging responsible for providing services to Senior American Indians in locating eligible individuals.
Evaluation Criteria: Three documented contacts to each organization a quarter.

8.3      Develop a fee-for-service arrangement with an American Indian Council, health, social service or governmental body to assist identification of persons who may be eligible by the Sixth Month of the First Grant Year.
Evaluation Criteria: 10% of the number served are American Indians as documented in annual reports of the grant.


OBJECTIVE 9  

Client Rights and Advocacy: Provide all referrals with information about the KRS Client Assistant Program and their toll-free number at time of first contact. Provide advocacy resource information.

9.1       Refer clients to Legal Aid, as appropriate, if CAP cannot resolve an issue.
            Evaluation Criteria: Client records reflect CAP information was presented. 
            No CAP complaints with a finding of fault.

9.2      Coordinate with the Kansas Advocacy and Protective Services Program. 
           
Evaluation Criteria: Client Exit Evaluations reflect satisfaction with services.

9.3      Coordinate with SRS Legal Services to refer clients needing a guardian ad litem.
Evaluation Criteria: All referrals will be made in ten working days of identification of need as documented in client records.

9.4       Immediately report possible elder abuse or neglect to the local SRS offices.
   
         Evaluation Criteria: All complaints or concerns referred in two working 
            days as documented in client records.

OBJECTIVE 10  

Peer sponsorships and visual support groups: Facilitate interactive opportunities with Peer Sponsors and role models to assist clients in adjustment to the changes in lives due to onset of blindness through Peer Sponsorship. Support the establishment of community-based Visual Support Groups.

10.1    Staff will recruit ten Peer Sponsors and match with clients who request peer sponsorships.
Evaluation Criteria: Documented ten Peer Sponsorships for Year Three, Four and Five.

10.2    Encourage Peer Sponsors to attend and provide leadership for Visual Support Groups, and assist in establishing new Visual Support Groups.
Evaluation Criteria: Documented attendance at Visual Support Groups by staff, Peer Sponsors and sponsored individuals.

10.3    Develop Visual Support Groups to assist, encourage socialization, to help facilitate the adjustment to visual loss through peer supports, and for educational purposes.
Evaluation Criteria: One new Visual Support Group per year beginning the Sixth Month of Grant Year One.

OBJECTIVE 11  

Program access and resource development: Promote stronger public and private partnerships to improve program access for older individuals who are severely visually impaired.  

11.1    Provide training and technical assistance, as requested, to the staff of the twelve Independent Living Centers. Contact will be made quarterly.
Evaluation Criteria: Technical Assistance Satisfaction Surveys and Training Evaluations are positive. Grant records document contacts.

11.2     Encourage the integration of the target population into community-based Senior Programs in existing local organizations or through information sharing, provision of technical assistance, and consultation.
Evaluation Criteria: Two community programs a year are made more accessible, integrated or provided with technical assistance as documented in grant records.

11.3    Work in concert with the Kansas Department on Aging and Area Agencies on Aging to increase accessibility to Senior Meal Sites and Senior Recreation Programs.
Evaluation Criteria: Two new programs are made more accessible each year as documented in grant records.

11.4    Work cooperatively with the Audio-Reader Program to present service announcements.
Evaluation Criteria: Two public service announcements each year.

OBJECTIVE 12  

Serving Tracking: Assure close monitoring of clients services and grant objectives for accountability and for quality and timeliness of service delivery. 

12.1    Develop an individualized written Senior Life Plan with each client using monitoring and reviewing schedules.  Each new plan will be reviewed and monitored by the Coordinator.
Evaluation Criteria: RSA Grant Manager satisfied with all grant reporting.  All Senior Life Plans are reviewed within one month.  The case record will reflect this review.

12.2    Refine the computerized record keeping procedures and update current forms to assure uniformity of documentation of services and accurate fiscal and program tracking.
Evaluation Criteria: Each case file contains uniform documentation.

12.3    Develop a tracking system documenting referrals, resource calls, public relations presentations, and provision of technical assistance.  All community development efforts will be documented in the Community Development Plan.
Evaluation Criteria: Procedures and forms developed and successfully used by the end of the first quarter.  All Objectives will be reviewed and any necessary corrective action taken quarterly. Contract Evaluator will monitor and recommend remedial actions to the Project Director.

OBJECTIVE 13  

Issues in Blindness Conference: Hold an Issues on Blindness Conference in Grant Year One, Three and Five. The goal of the conference will be to facilitate common understanding and interaction among professionals regarding aging and vision, and to encourage collaboration to develop better service delivery systems.

13.1    Work cooperatively with RCEP, the sponsor of the conference, and encourage collaborative sponsorship from the Kansas Department of Aging.
Evaluation Criteria: Conference support and funding diverse. Positive conference evaluation forms.
